The problem lie deeper, in defining what is hard in an mmo, and each mmo got it's own viewpoint on this question.
What is seen as hard by players, and what is seen as hard by developers is different as well.
We can take as example the recently 'released' Skyforge.
In this game they have those auto-attacks that do insane damage to you and cannot be avoided. Some may see this move as difficulty just as the devs no doubt intended.. i however see this as a cheap way out of having to create actually complex combat mechanics based purely on no-target combat.

One thing i dislike more than all else though, theme-park mmos that offer no challenge whatsoever. They just take your hand and you stroll through completely predefined park holding hands together maybe even laughing merrily.. right until you get bored next week.
Then it's love no more. :>

Ever since the advent of "mobile gaming" more and more of these chodes are popping up, whining a game is too hard and getting it nerfed to hell to where even an elementary school child can play it efficiently. Games are becoming pathetic with this "me too" mentality.

I think there needs to be a mix of games for players. An easier game for the more casual while a hard game for those who need more of a challenge. No game can please everyone.

I very much like the lot of later content in a game to be in between moderate difficulty and hard. I think devs need to put more heart and soul into creating the perfect balance now adays. I see alot of games go all wrong due to content being a boring unengaged grind, or bash you face of your desk difficult. I suppose thats just preference tho. Idk.

As far as difficulty goes, I think combat should require paying attention, as in movement, interrupts, things of that nature. Any dungeon should carry with it the possibility of a wipe and require the team going through it to actually work together. If people aren't willing to do that and would rather have fight where 'standing in the fire' means nothing, then frankly, why bother playing at all, maybe you should just go autoquest to max level.
